PFACQS® 2.0 Toolkit
This toolkit, which includes a comprehensive Assessment Tool, was built by H2Pi and leveraged with Vizient, Inc. to support the Centers for Medicare and Medicaid Services (CMS) Healthcare Improvement Innovation Network (HIIN).
PFACQS® 2.0 uses a Logic Model as a theoretical framework to explain the contribution and impact of Patient and Family Engagement (PFE) in improvement work. Illustrated below, the Logic Model visually displays the influence that robustly implemented PFE has on strategy and organizational culture, as well as outcomes, when PFE is integrated into quality and safety programs.
During the analysis, three primary drivers emerged that contributed to the success of PFE programs and improvements in outcomes from high-performing PFE hospitals.
